I managed to get to the end of the cave, to the bright light past the wall of ice, but after the loading I found myself back at the cave's entrance, with all my progress lost.

I have a blank spot in my inventory for item 2. Can someone tell me what it is and where I can find it?

I was in the huge stone building with the numerous passageways and levels and was searching for the third star to place on a door to further my progress when I fell into a lower level. I died; however, when the game reloaded I was permanently fused into a wall. I reloaded the game from that point, but each time I started to walk I clipped into the sky. Really buggy. As you first arrive to the city, the character no-clips through the ground right in front of the entrance of the first accessible building, and falls out of the map. It happens every time, it's a permanent bug.
